Beginning with the 2006/07 school year, all 6th-12th
grade Covenant students who participate in athletics
(Volleyball, Flag Football, Basketball, Cheerleading,
Baseball, or Softball) are required to complete the
FHSAA Physical
Evaluation Form. The first page is to be completed
by a parent and signed by both the student and a parent.
A Physician or Nurse Practitioner must complete the
second page.
The completed form must be submitted to the school
office prior to the first day of official practice for
the sport.
The Bay County Medical Association will provide free
athletic physical for all Middle and High School
students on Tuesday, August 1st from 6:00 p.m. to 8:00
p.m. at the Marina Civic Center. If you would prefer,
you can have the form completed by your family medical
professional.
