Admission to CCS
To initially enroll a student in CCS, parents/guardians must submit:

• a completed, signed application
• unofficial copy of transcripts, attendance records, standardized test scores, and any appropriate school records from the last two (2) years
• a completed, signed Parent Interview Form
• original Florida Department of Health Blue Form (proof of immunizations)
• original Florida Department of Health Gold (or Tan) Form (proof of health exam in Florida)
• a completed, signed & notarized CCS Permission Form (CCS has a notary on staff)
• a non-refundable registration fee
• a non-refundable deposit
• a copy of the student’s birth certificate
• a copy of health insurance card
• FACTS form (payment method)

Class size is limited and a place cannot be held for any child without receipt of the above items. Parents of new students shall meet with the headmaster before an application can be approved.

Students are considered for enrollment at CCS without discrimination based on race, color, national or ethnic origin.

Florida Health Requirements

• Documents must be originals, photocopies and facsimiles are not acceptable.

• Florida Department of Health "Blue Form" (proof of immunizations).

• Florida Department of Health "Gold Form" (proof of health exam in Florida).
